| SHE01 | Upon review of the 2026 TPZ IMS Internal Audit Plan, it was noted that the SHE Department was scheduled to be audited once during the year. The auditors requested the matrix used to determine audit frequency, and upon review, it was identified that SHE falls within the category requiring two audits annually. This discrepancy arose from an incorrect EMS rating, which was recorded as 2 instead of 3, resulting in a total score of 16 instead of 17.
